Intermediate Care is all about helping people in and out of Hospital so they are fully cared for when they need it. It is about making sure that all of the people that we rely on for our health and care, are always working together effectively to make sure that we get the care and support that we need, when and where we need it.
Since October 2008, almost 500 doctors, nurses, patients, carers and local organisations have worked together to help develop and draft these proposals. Over 12,000 people could directly benefit from the new services which aim to radically improve health and wellbeing by providing easy access to more care at home and in the community.
Now that we have developed these proposals, we need your feedback to tell us what you think.

You don’t need to be a health expert or an expert on local services. We want to hear your ideas, opinions, concerns and questions about the proposals, so we can get this service right and ready to launch in 2010.
