Achievements since April 2002

  • Significant organisational development.
  • Awarded lead for Teaching PCT status for sector.
  • GP Specialist training commenced.
  • Application approved for Local Improvement Finance Trust (LIFT) status.
  • Proposal developed for Diagnostic and Treatment Centre.
  • Locality Forums established.
  • Professional Education Forums developed; GP, Practice Nurse, Practice Manager, Community staff.
  • Diabetes patient education programme piloted, a proportion of the participants going on to train as "Expert Patients" via the Government programme.
  • PCT led PMS for refugees and asylum seekers.
  • Coronary Heart Disease (CHD) community rehabilitation programme established.
  • Primary Care Collaborative - Advanced Access programme rolled out to practices & Advance Access facilitator recruited.
  • Medicines Management Collaborative application, August 2002, to include five practices across Brent.

  • Tackle inequalities both for the wider determinants of health and access to services.
  • Extend specialist care provision for the homeless and refugee's through PMS (Personal Medical Services).
  • Be responsive to patients.
  • Implement a wide ranging Health Promotion Strategy.
  • Expert patient and patient education.
  • Evidence based care delivered by appropriately trained staff.
  • Improve immunisation and screening.
  • Extend Partnership working.
  • Insure accurate needs assessment through information strategy.
  • Develop a comprehensive Primary Care Strategy.


Tasks/Goals 3-5 years

  • Bring services nearer to the patient.
  • Develop an estates strategy to provide facilities to enable more effective local care.
  • Roll out GP Specialist training - establish Diabetes & CHD programme & roll out to other areas such as Dermatology, ears, nose &Throat (ENT) and Orthopaedics.
  • Create Primary Care led chronic disease management models for diabetes and CHD including specialist nurses.
  • Have community team lead midwifery services.
  • Implement National Service Frameworks.
  • Effectively deliver the NHS Plan.
  • Implement an electronic booking system.
  • Enhancing patient choice, though extended provision of services.
  • Deliver a skilled and stable workforce.
