What Information We Collect About You
To ensure that you receive the best possible care from us, the PCT hold important records about you. These records may be written down (manual records) or held on a computer (electronic records). These records may include the following information:
- Basic details about you, such as name, address, date of birth, NHS number and next of kin.
- Contacts we have had with you, such as clinic visits.
- Notes and reports about your health and any treatment and care you need.
- Details and records about the treatment and care you receive.
- Results of investigations, such as X-rays and laboratory tests.
- Relevant information from other health professionals, relatives or those who care for you and know you well.
Ethnicity information is also collected to allow us to ensure that we are providing an appropriate and accessible service for the community by:
- Assessing who is using the service.
- Identifying the health needs and patterns of illness amongst different ethnic groups.
- Highlighting any gaps in the service provision for a particular community.
- Developing staff awareness of and response to individual needs of different ethnic groups within the community.
You do not need to provide us with ethnicity information if you strongly object. |
