Community Children’s Nurses

Community Children’s nurses are qualified registered nurses who have been specially trained in the care of sick children and their families. They give skilled nursing care and promote health and well being for children in the community. They enable sick children to remain in their own homes, avoiding hospital admission or facilitating early discharge from hospital.
The Community children’s nurses offer a continuing care nursing service for children with very complex medical needs who require a home nursing package. They also provide a specialist palliative care service for children with life limiting conditions.
The service is available to children from birth until 16 years of age (up to 18 years of age for children with special needs). Hours of service: 8am to 4pm daily including weekends and bank holidays.
You can contact the team for confidential advice or an appointment through your child’s GP, health visitor, school nurse, paediatrician or phoning direct.
