BECaD Diabetes Model
2006 is going to be exciting year for health care in Brent. The BECaD project has gone live in March 2006 and with it come changes that will affect service providers and users. This is particularly apparent for those involved in one way or another with diabetes.
Owing to a higher than average incidence of diabetes in Brent there can be very few health care professionals in this area who do not have some aspect of their everyday workload that incorporates something to do with diabetes.
The decision of some of the newly formed practice based commissioning (PBC) groups to make diabetes a priority reflects and acknowledges the importance of this potentially serious condition.

* Majority of patients will have continuous screening & assessment in primary care (see model in orange) * Primary care will be able to refer anyone that falls outside of their realms of expertise to the Single Point of Access (SPA) : see model in blue * The SPA will act as an interface between primary care and the acute trust * One referral form is only needed for all services (eg. dietician, education) that can be organised by the SPA team * Patients can be seen within a week in rapid access clinics run by consultants and/or other members of the multi-disciplinary team * In addition, instant (within 24 hours) replies by telephone, email and fax for any queries that are received by SPA * Only specific group of patients are seen at the Expert Consulting Centre in the acute trust (see model in pink) For further information contact:
