Intermediate Care is all about helping people recover from illness and maintain their independence by ensuring they are fully cared for when they need it. It is about making sure that all of the people that we rely on for our health and care, are always working together effectively so that we get the care and support that we need, when and where we need it.

Since October 2008, almost 500 doctors, nurses, patients, carers and the organisations that represent them have worked together to help develop draft proposals for new Intermediate Care services in Brent.

Over 12,000 people could directly benefit from the new services which aim to radically improve health and wellbeing by providing easy access to more care at home and in the community.

STARRS is an enhanced service from NHS Brent and Brent Council that will give older people access to a new range of intermediate care services at home or in community care. STARRS offers more personalised services that promote independence and radically improve health and well being by:

   Providing new services to protect people from unnecessary Hospital admission
   Providing more effective rehabilitation services to enable early discharge from hospital
   Providing more effective reablement services that help people to ‘do’ more for themselves and protect them from unnecessary admission to long term residential care.

This affects all Brent residents and it is crucial that you have your say now!
