The Community Health Team
The Community Health Team are a diverse multidisciplinary team of health care professionals, providing a range of primary care services, in addition, assessments, support plans and services to meet the needs of adults with a learning disability. The Community Health Team also works with young adults making the transition from children’s services and education.

Who are the team members?
Assess and treat psychiatric needs including medication reviews
The Psychology team work for people with a learning disability by firstly assessing whether a person has a learning disability and looking at what their needs may be. The team then encourage them to see a social worker to find out what help is available for them.
Secondly, the team will work with clients in therapy to help those overcome problems, help them develop understanding and help them adapt to changes.
Thirdly, the team work with the people who support the clients. This can be either through training, support or advice to families and care staff.
- Speech and Language Therapist
Speech and Language is about supporting people to communicate better. This may be through speech and or additional methods such as pictures, symbols, objects and signing. A great deal of work involves training staff in these methods. In addition they also deal with people with eating and drinking difficulties
The team is responsible for developing and delivering a Community Physiotherapy Service for adults with learning disabilities in Brent (except the Kingswood Centre and associated units).
The Physiotherapist will carry out a comprehensive assessment and form an individually tailored management plan. This involves treatment, advice, education, training, multidisciplinary work, provision of equipment and monitoring orthotic and wheelchair needs. They also provide training and supervision to trainee physiotherapists on clinical placements
- Loss and Bereavement Therapy
The therapist provides counselling and psychotherapy to people with varying degrees of learning disabilities experiencing loss and bereavement. The aim of the therapist work is to find approaches that facilitate a client’s engagement and enables concrete exploration of the unique grief that is experienced by the client.
Offers support to those who present behaviours which are challenging to the client, carer, staff etc. CIT also offer comprehensive assessments, behavioural plans, guidelines and risk assessments. Training is also provided to anyone who supports people who present challenging behaviour, Autism Awareness and Personal centred Risk Assessments.
The Community nursing team consists of 6 nurses. They take a collaborative person centred approach to meet the needs of people with learning disabilities who they provide a service to.
The specialist areas covered within the team include mental health, epilepsy transition, complex needs, challenging needs and health promotions
The roles include assessing, teaching and developing positive health behaviours and personal growth. They take a therapeutic role in the prevention of ill health and deteriorating health, a coordinating role in the planning delivery.
Epilepsy Nurse
“Incidences of epilepsy in people with learning disabilities is higher than in the general population”
The role of the epilepsy nurse is to help manage epilepsy to improve their quality of life and reduce health inequalities. Training is also provided aimed at clinical and professional staff from Brent NHS as well as parents and relatives of people with learning difficulties who have epilepsy. |
