Community Matron Service

Incorporate within the District Nursing Service, the Community Matron Service provides very high standard of individualised, holistic case management for patients with complex co-morbidities and polypharmacy whose day to day living are affected by these conditions

The Community Matron Service works very closely with other multidisciplinary teams to ensure that this group of patients are kept well at home, preventing unnecessary hospital admissions and reduce emergency bed days

Community Matrons are highly skilled advanced nurse practitioners. They are independent/supplementary prescribers who work closely with GPs to manage patients’ conditions proactively to keep them safe at home. Community Matrons ensure that patients care is well co-ordinated to provide seamless service to all patients.

They provide advice, education and support to patients and their carers in their own homes. They also provide teaching sessions to nurses and other professionals.

Referrals are accepted from GPs, other agencies, families and patients. Fax referrals to the District Nursing teams and mark for the attention of the Community Matron.
