District Nursing



The District Nursing Service provides health and nursing care to patients closer to home, to enable people to remain in their own homes, prevent unnecessary hospital admissions and A & E attendance. District Nurses provide support and teach patients and carers to achieve independence and self care. Patients with complex health needs are case managed by advanced practitioners.

The service receives referrals from health and social care professionals; patients need to be adults 16+ with clear needs for skilled nursing intervention. There is no waiting list, but there are criteria for prioritising visits and assessments.

District Nurses are skilled nurses and are committed to treating patients in their journey through the health care system with respect and honesty, ensuring that good practice is based on relevant, up to date skills and expertise.

The Service operates 24 hours, 7 days per week. As of the 1 April 2011, 5pm the out of hours service for District Nursing can be reached on 020 8967 500.
